Media Release: I give Horus Academy permission to photograph and record my child during lessons, recitals, and school events, and to use these photos and videos on the Horus Academy website, social media, and promotional materials. No full names will be published. I understand this is optional, and I can withdraw consent at any time by emailing info@horusacademy.com.

Make-Up Policy: I have read and agree to the Horus Academy Make-Up Lesson Policy. I understand that: • Make-up credits are only issued for private lessons or band sessions that I cancel ahead of time through the Noto Parent Portal. • Make-up classes take place on Saturdays in the form of group sessions and are booked through the Make-Up Sign-Up Form, with up to 6 students per time slot. • Each make-up credit expires one year after the date of the cancelled lesson. • Horus Academy may deny a make-up credit if the lesson was not cancelled in the portal.*
